At GCM, we are able to provide Geriatric Care Management Services on a number of levels. All of our services begin by completing a Comprehensive Assessment that establishes a baseline of essential information. We act as Advocates through our Care Coordination Services and can remain involved on an on-going basis through our Continuing Care Program. Whichever service is chosen, we document important information needed for optimal care and keep track of changes so that information is up-to-date. We also serve as Durable Health Care Power of Attorney or Court Appointed Guardian.

Our Geriatric Care Managers will always go to where the client is (i.e. at home, hospital, assisted living, residential care home or nursing home) to more accurately determine both immediate and long term needs.
