Individual & Family Consultations

Consultations are usually conducted in our office.  They provide an opportunity for families to consult with a professional to discuss specific concerns, provide history and life context, problem solve, plan ahead and identify specific recommendations for the care of the client.

Consultations are also an opportunity to discuss and resolve conflict with the ultimate goal being to pursue a mutually agreed upon plan of action. Adult children are often trying to find solutions to what each sees as the needs of their parent. Finding a balance that meets the older person’s needs while at the same time recognizing their individuality and years of independence, is a delicate situation that deserves time and careful consideration.

Consultations often evolve into a decision to utilize a professional Geriatric Care Manager in the coordination of their loved ones care.  In such instances, our professional geriatric care manager makes a home visit, at least once each month in order to monitor the provision of care, evaluate changes in care needs coordinate services, and advocate for the client, as needed.

*The Initial Consultation does not and cannot take the place of an Initial Assessment and is intended for families of clients and not the clients themselves.
