Our comprehensive assessment gathers information needed to evaluate immediate, short term and long term care needs. The assessment may include obtaining and evaluating information on:

  • Past and Present Medical History & Medications
  • Memory Status
  • Behavior Concerns
  • Nutrition
  • Typical Daily Activities
  • Quality of Life
  • Emotional/Mental Health
  • Reasoning Ability & Judgment
  • Physical Functionality
  • Living Environment
  • Safety
  • Ability to manage Medications

This comprehensive assessment is completed by one of our professional geriatric care managers. The client and family’s concerns are addressed at this time.  Our goal is to identify what is needed to ensure the client’s health, safety, and overall wellbeing and determine the cost necessary to accomplish this.   At the time of the assessment, the geriatric care manager provides verbal recommendations, which take into account affordability and the wishes of the client.  

Many families also prefer a typed report, which summarizes the assessment and includes more specifics on the recommendations.  GCM terms this the “Medical Care Summary”.  Having this information in one comprehensive document serves as a reference point for families and health care professionals.  The assessment and recommendations can be presented and discussed in a follow-up meeting.    

*Reassessment will be done if more than 1 year has passed since the initial assessment was first completed.
