Geriatric Care Management A Limited Liability Company (GCM) has been servicing the New Mexico community and families from around the U.S., since 1994. In our 15 years of integrating services for seniors, our goal has been to maximize each person’s level of independence, while ensuring and enhancing their quality of life.

MISSION STATEMENT

Geriatric Care Management recognizes, values, and respects the unique qualities of each aging person, evaluates their needs, and locates the best resources to maximize that person’s level of independence, while honoring individual wishes and values so that the most informed decision will be made. 

GCM enhances decision-making, choice, and affordable solutions, in determining and obtaining the type of care that is needed and can stay involved in order to coordinate care on a continuing basis, making adjustments as needed, and avoiding crises

At GCM, we provide Care Management Services on a number of levels. Most of our services begin by completing a Comprehensive Assessment that establishes a baseline of essential information in order to develop a personalized care plan. We serve as Advocates through our Care Coordination Services and can remain involved on an on-going basis. We can also act as Durable Health Care Power of Attorney or Court Appointed Guardian. 

Through our experience, we have found that process is as important as outcome. It is very important not to rush decisions.  Because of work and family obligations, many families feel forced to make quick decisions about what should happen to meet their parents' care needs. However, what elders dread and resist the most is change!  GCM respects the time it takes to accomplish what matters and what is needed in the elder’s daily life. 

This is a core benefit of utilizing Geriatric Care Management. By identifying the specific resources that are needed on an individual basis, we will help you save time by not having to review lists, make unnecessary phone calls, or feel overwhelmed by not knowing where to look for what you need. Our experienced and professional guidance facilitates the decision-making process. Our Geriatric Care Managers will always go to where the client is (home, hospital, assisted living, residential care home, or nursing home) to assess immediate, short-term, and long-term care needs. Most importantly, we keep family members informed of all changes, whether local, out of state, or out of the country. The key to all of our services is communication and coordination!
